This position provides concrete and supportive services to hospitalized patients and their families, including psychosocial assessment, individual and/or family counseling and crisis intervention.  Provides Discharge Planning Services to achieve hospital goals of Quality Care and cost containment.  Participates in Hospital Team Conferences.  Acts as a liaison to relevant community agencies. 

Provides Case Management Services, both within the hospital and in the community, to a capitated patient population with responsibility for facilitating access to care, coordinating care in medically complex situations and appropriate utilization of financial resources.  May provide social work student supervision, in-service presentations in areas of social work expertise for hospital and community agencies, and may facilitate groups for staff, patients or families.  This position treats/cares for patients of all ages.

Job Requirements

  • Master’s degree of Social Work required
  • LCSW required
  • Minimum of one (1) year post-graduate experience in a Clinical setting (Health Care Facility)
  • Current BLS Certification

Salary: $51.00 – $56.00 per hour
