Transitional Care Unit
The Transitional Care Unit (TCU) at Oak Valley Hospital offers skilled nursing and rehabilitation services for patients who are recovering from an acute illness, injury, or surgery but are not yet ready to return home or to an independent living environment.
Located within the hospital, the TCU provides short-term, post-acute care in a medically supported setting. Patients benefit from seamless access to the hospital’s full range of services, including diagnostic imaging, laboratory testing, and medical specialty consultations.
Our goal is to support a safe recovery and smooth transition back to daily life, helping each patient regain strength, independence, and confidence through personalized care and therapy.

The Transitional Care Unit (TCU) at Oakdale Nursing and Rehabilitation Center (ONRC) is designed to provide a comfortable, welcoming environment that supports both healing and social connection. The unit features a dedicated dining room, a patio area, and a common space where patients can relax, enjoy music or television, and socialize with family, friends, and fellow patients.

Each patient room is equipped with a private phone and television, offering personal comfort and convenience during their stay. To support rehabilitation, the TCU also includes a fully equipped therapeutic gym, providing the space and tools necessary to help patients achieve their individual recovery and mobility goals.
